The Berlitz Česká republika spol.s r.o. (hereinafter as “Berlitz“), in sense of the Act no. 101/2000 Coll., of Personal Data Protection, as subsequently amended (hereinafter as “APDP”), has the honor to inform you of processing your personal data by Berlitz and of your rights. Berlitz hereby notifies you that it is processing/will process your personal data to the extend provided by you in connection with your application for a contractual or other legal relationship and/or any other contractual or other legal relationship between you and Berlitz, or the data that Berlitz collected otherwise to process them in accordance with Czech law for the following purposes: the purposes included in your consent given to Berlitz, your negotiation over the contractual relationship, performance of the agreement, your serious interests protection, personal data publication, Berlitz rights protection, archiving administered by law, offers of trades or services made to you, our customer. Your name, surname and address transfer for the purpose of offering trades and services under s. 5, p. 6,7 of APDP, unless prohibited by special law. Berlitz shall inform you that upon filing your written application you have the right, based on the conditions specified in the provision of s. 12 of the Act 101/2000 Coll., of Personal Data Protection, as subsequently amended, to access the information of the processed personal data of yours, and other rights stipulated in s. 21 of APDP, in particular the right to appeal, in case of breach of our duties, to the Personal Data Protection Authority (www.uoou.cz) for redress enforcement and to claim that Berlitz shall make the necessary steps under the stated provision of APDP.
	Having understood the above policy, I authorize the treatment and communication of my data by Berlitz.
